Geriatric Care Planning
Geriatric care planning is necessary because the health care system is fragmented at a time when the deluge of geriatric health needs is increasing in complexity.
A comprehensive assessment and plans involve information gathering, attention to immediate concerns along one or several dimensions, and management of the health and social service’s systems for the elders and families benefit.
The plans are unique to the people but the systems involve the same biological, psychological and social dimensions all over the country.
The Seacoast systems are well-known to me, after 40 years of geriatrics in community hospitals and I have trusted partners to collaborate on geriatric care plans.
